ISSUING DUPLICATE COURSE COMPLETION MATERIALS

CST understands that we are all human and sometimes things get misplaced. Below is the process and cost for issuing duplicate course completion materials.

For OSHA & SST Replacement Cards: 

All students must call prior to picking up their cards to verify course and date of completion, and must be paid in full prior to printing, or ordering the cards from OSHA.

Replacement card fee breakdown:

OSHA 30: $60 per card 

DOB SST 10/32: (obtained from CST): $100 per card

DOB SST 10/32: (obtained from another Course Provider): $100 per card

All other cards (Scaffold, rigging, flagger, etc.) $60 per card

Once we receive payment, all cards will be available for pick up at our Long Island City office. Please bring your government issued ID (must match the learner records).

CLOSED

OSHA has a 2-8 week window to send replacement cards to our office. We will provide a temporary certificate which is valid for 90 days.
